I'd read somewhere that you needed to use the sword techniques to prevent the scripted insta-death which seems to bypass the auto-revive items. So when I fought him (multiple times) I cycled through all four techniques, one each turn. Used a lot of mana but seemed to work. Once I started using that strategy the second form never insta-gibbed me again.

For the third form. Once he puts up the red sword wall, if you attack the following turn you die. After the first turn you seem to be safe. Once he moves to the front of the sword wall, all bets are off. He just seems to insta-kill you repeatedly regardless of what you do. It was then that I gave up.
