Zelena the Wicked Witch first did her melting bit quickly when she got water splashed on her by Dorothy, but apparently it was a ruse and she recovers.

Then, after she's defeated by the main group, Rumple, free from her control, stabs her with his dagger. She turns into a life-sized porcelain doll, then crumbles to dust. Whether she's officially dead doesn't seem certain but it's looking that way.
