Only for MtF they can't. FtM, the hormones do cause their voice to break and drop like a boy's.

From what I've heard with special training a male voice can sound female anyway.

As for the bone structure: nothing extensive surgery and (in the future) tissue replacement therapy can't fix.

Though I myself like being a guy, it's good to see people pleased with their new bodies.
