resiz3d has some good ideas there so I'll add just a few others...

You have no reason not to shoot it. Besides you imply she has a body thats worth looking at especially if revealed slowly as the clothes stretch, shred & tatter. If you think to place an ad ahead of time(Craigslist) and its a non-totally nude type growth many actresses would do that very cheaply, for free or a simple deferred but-still-not-all-that-large sum you can give her after its released. There are tons of models and actresses looking for paid work who will take unpaid work simply for the credit. Just act like a professional while you shoot and don't mention the word "fetish".

When it comes to shooting remember that you have 100% control over what the viewer sees and when they see it. If you are that worried about it make sure the face is not shown in any stills and be creative with your shot choices especially in the beginning of the clip.

Here's a few stills I grabbed off a BE clip I am working on. I know Taylor's a pretty girl but if you didn't already know what she looked like you couldn't be 100% sure of that...





and here's a shot from Tainted Flintstones that establishes her height in relation to the painting without showing her face.


If the face is objectionable as you say then DON'T SHOW IT RIGHT AWAY because every other shot after that without her face in it the viewer will associate the two and possibly lesson their enjoyment of the clip.
