Just about everything by Jack L. Chalker has transformed characters, though I'm not sure if he wrote any detailed transitions, I've only read a few of his books.

I've read a fair amount, and there really isn't a lot of what this forum would define as "process"; most of his transformations are of the "poof" variety, or nearly so. But there is a lot of the transformation subjects attempting to deal with their new forms/new worlds/new political structures/new whatever.
